<strong>Hydrogel</strong> is a permanent soft tissue filler. It is made from 5% polyacrylamide gel and 95% water. The cross-linked Polyacrylamide forms a real gel that is very similar and highly bio-compatible to human soft tissue. The gel is injected beneath the skin or muscle (depending on different places) where it can&#8217;t be seen or felt and remains in place permanently. One week after injection there will form a very thin membrane around the implant which helps to keep it in place. It moves with facial expressions because it&#8217;s very elastic. It is easy to inject, permanent effect, and can be removed at any time after injection.  During the past 6 years since 1997, more than 20 million ml of hydrogel has been used to treat over 300 thousand patients. Since first launch in 1997, PAAG has been used all over the world, including Asia, Europe, United States, South America, etc. A large number of animal tests and clinic studies done by big government hospitals and universities proved that the Medical Polyacrylamide Hydrogel(PAAG) is a safe, ideal and effective soft-tissue filling cosmetic material, because it is high inertia, good biologic compatibility, no chemical reactions within human body.</p>

<p><strong>What other type of permanent fillers are there?</strong> <strong>Silicone</strong>. Several forms of liquid injectable silicone are FDA-approved for ophthalmic but not cosmetic uses. The American Society for Aesthetic Plastic Surgery has warned against the use of liquid injectable silicone for cosmetic purposes pending further investigation. Unfortunately, silicone oil injections can cause granulomas and long-term inflammatory changes and can migrate. Several series and anecdotes report the use of liquid silicone for HIV-related lipoatrophy, building on experience with use in other forms of lipoatrophy. Given the potential risk of silicone relative to alternative agents, it is not a preferred treatment option. Studies evaluating the risk versus the benefit of silicone oil in HIV-associated facial lipoatrophy are ongoing.</p>
<p><strong style="font-size: 14px; line-height: 1.5em;">Expanded Polytetrafluoroethylene (ePTFE) Implants (Gore-Tex/Gore SAM, SoftForm).</strong> These FDA-approved implants are more appropriate for correction of significant facial abnormalities. They can be inserted using a small incision while the patient is under local anesthesia. There are potential problems with lumpiness and physical obviousness of the implant. In general, cosmetic use of ePTFE implants is reported to cause adverse events, including movement of the implant, infection, swelling, induration, and uncommonly extrusion (emergence of the implant through the skin).</p>
<p><strong>Calcium Hydrogel (Radiance)</strong>. Preparations based on calcium hydroxylapatite (CaHA) microspheres suspended in carboxymethylcellulose gel were first FDA-approved for vocal cord injections and for use in a range of orthopedic surgery approaches and in radiology (CaHA is radiopaque) but have also been widely used for treatment of wrinkles and lip augmentation. CaHA is bioabsorbable over several years. Reported problems include hardening and overcollaginization requiring corticosteroid injections. Limited data exist regarding its use in HIV-associated lipoatrophy. Polymethylmethacrylate (Artecoll/Artefill).</p>
<p><strong>Polymethylmethacrylate</strong> (PMMA) in smooth microspheres suspended in bovine collagen has also been approved by the FDA. The bovine collagen in the implant is reabsorbed over time, but the presence of the PMMA microspheres generates new collagen production in that same site. PMMA has been used in Europe for close to a decade and is available in other countries. A large series in Brazil described its use in HIV-associated lipoatrophy, although clear systematic collection of safety data was lacking. Side effects reported included postinjection swelling and pain. Ultrasonographic data indicated an increase of dermal thickness sustained up to 5 years.</p>
<p><strong>Polyacrylamide gel</strong> <strong>(Aquamid, Contura, Bio-alcamid).</strong> These agents are not FDA-approved, but they have been used in clinics in Europe (where they are approved) and Latin America. Reported data consist mostly of large clinical series accompanied by impressive photographs. Some systematically collected data have been reported with Bio-alcamid. The different polyacrylamide gel products have similar characteristics. They are relatively viscous and are injected subcutaneously. Some authors suggest that use of a dental block is advisable to limit discomfort at the time of facial injection. Following injection, the substances encapsulate, enabling at least partial removal, such as with a large-bore needle, at a later time. This makes them of interest for use in a condition that may partially resolve over time. In a case series of 73 HIV-infected patients with up to 3 years of follow-up, &#8220;excellent&#8221; aesthetic results were reported, with no episodes of implant dislocation, migration, granuloma, or allergic reaction recorded.</p>
<p><strong>What are the risks involved in having Hydrogel Buttocks injections?</strong></p>
<p>Most patients will recover smoothly after Injections surgery. But as in every medical procedure, complications are always possible. These instances are uncommon and are virtually always treatable.</p>
<p><strong>Possible Complications and reactions to be aware of:</strong></p>
<ul>
<li>Injection area damage</li>
<li>The skin around the injection could become infected</li>
<li>The body filler can be absorbed</li>
<li>Painful allergic reaction</li>
<li>Immunity issues, if incorrect (black market) filler is used</li>
<li>Swelling</li>
<li>Redness</li>
<li>Pain</li>
<li>Tenderness and warmth in the butt area</li>
</ul>
<p><strong>Infection is a possibility with any buttocks Injection.</strong> <strong>Reactions to anesthesia or pain medication</strong></p>
<ul>
<li>Advise your doctor or anesthesiologist if you had ever experienced a reaction during a previous procedure.</li>
</ul>
<p><strong>Going Home after Buttocks Injections</strong> <strong>Discharge instructions</strong></p>
<ul>
<li>Your Doctor will give you a set of guidelines of what to do and expect over the first week.</li>
</ul>
<p><strong>Recovery</strong></p>
<ul>
<li>A full recovery is expected in three days</li>
<li>You will be given leave to go home about one hour after the procedure.</li>
<li>You must take it very easy the first three days and avoid sitting directly on the injection area</li>
</ul>
<p><strong>Medication</strong> <strong>Resume medications</strong> you were taking before your procedure, unless directed by your Surgeon. As pain decreases, take Tylenol . <strong>Your Surgeon may prescribe medication</strong> although for buttocks Injections this is not very common. <strong>Activity Physical Guidelines</strong></p>
<ul>
<li>Follow the physical activity guidelines instructed by your doctor.</li>
</ul>
<p><strong>Buttocks precautions:</strong></p>
<ul>
<li>Limit sitting in the injection area to an absolute minimum for the first three days.</li>
<li>Sleep on your Stomach . Even for laying in bed to watch television, lay or your side or stomach</li>
<li>Watch for signs of infection and swelling.</li>
</ul>
<p><strong>Bathroom</strong></p>
<ul>
<li>Avoid Public toilets. Use sterilized toilet in your home when possible for first three days</li>
<li>Avoid sitting down when going to the bathroom</li>
</ul>
<p><strong>Stairs</strong></p>
<ul>
<li>Avoid any large amount of stairs for the first two day as this puts pressure on your glutinous muscles .</li>
</ul>
<p>Sports</p>
<ul>
<li>No participate sports during the first three days after getting your body filler</li>
<li>No exercise equipment, or spas until your doctor tells you it is okay.</li>
</ul>
<p><strong>Sexual activity</strong> After buttocks Injections, most people are concerned about resuming sex.The general rule is until you are cleared for sports by your doctor, having sex is not recommended.</p>
<p><strong>Driving</strong></p>
<ul>
<li>No driving for at least two days.</li>
</ul>
<p><strong>Care of your wound</strong></p>
<ul>
<li>If your injection area was closed with sutures you will need the doctor to remove them</li>
<li>Wash your incision area with only mild soap and take care to pat it dry</li>
<li>Do not take a bath.</li>
<li>No lotions on your incision unless directed by your doctor</li>
</ul>
<p><strong>Signs of infection-what to keep an eye out for.</strong> Each day with your shower, Check the injection area for any of these signs of infection:</p>
<ul>
<li>Redness</li>
<li>Drainage</li>
<li>Sudden Swelling</li>
<li>If you see any of these signs, call your doctor ASAP.</li>
</ul>
<p><strong>Antibiotics</strong></p>
<ul>
<li>Are generally no prescribed for body fillers and body injections in general</li>
</ul>
<p><strong>Swelling</strong></p>
<ul>
<li>Swelling ( edema) is to be expected after any procedure, this is common and should go down in a few hours to one day for this procedure.</li>
</ul>
<p><strong>Follow-up</strong></p>
<ul>
<li>If sutures were used for your injection, these will be removed in three days normally</li>

As a result many of our plastic surgeons specialize in buttock implants and buttocks shaping and contouring and pride themselves on creating the perfect buttock!</p>
<p><strong>How do I get a fabulous, sensuous and rounded buttock?</strong></p>
<p>The great news is you have options and even combinations within those options!<br />
There are two techniques used to create curves where you have none.</p>
<p><strong>At a Glance</strong></p>
<p><strong><span style="text-decoration: underline;">Buttock Augmentation with Fat grafting technique</span></strong>&#8211; This technique uses your own fat from the abdomen or thighs using Liposuction. The fat is then processed and then used to sculpt and contour your buttocks to the desired shape and size.</p>
<p><strong><span style="text-decoration: underline;">Buttock Augmentation with Implants</span></strong> – This technique uses a silicone buttock implant (different shapes, sizes and textures) to create a rounder and fuller shape. Fat grafting and Liposuction is commonly done in conjunction with this procedure to create the contour and shape desired.</p>
<p><strong>Buttock Augmentation with Implants</strong><br />
<strong>Are a you good candidate?</strong></p>
<p>A good candidate for a buttock implant:</p>
<ul>
<li>is in good health</li>
<li>is a non-smoker</li>
<li>is willing to follow all recovery recommendations</li>
<li>has realistic expectations</li>
</ul>
<p>An ideal candidate for any type of cosmetic surgery will understand that cosmetic surgery is intended as a means of enhancement. A Buttock Augmentation / Enhancement with implants can have very dramatic results depending on the patient, but individuals should be realistic about the results they can achieve through surgery.</p>
<p>You are also at increased risk for complications if you have diabetes, poor circulation, heart, lung or liver disease, smoke, have a family history of blood clots, take certain medications, etc. You&#8217;ll want to discuss your medical history thoroughly with your physician before you choose to undergo surgery.</p>
<p><strong>The Initial Consultation</strong></p>
<p>The surgeon will request digital photos to be sent and for you to fill out our medical history form. Then we can arrange an online consultation. Your doctor will ask you to describe in detail what you would like improved. Be specific about what you would like done. Bring up all of your concerns no matter how small. This is your time to discuss your wants and desires so your surgeon fully understands your expectations. Now that your surgeon knows what you expect he&#8217;ll be able to determine whether your goals are realistic.</p>

<p><strong>What decisions need to be made?</strong></p>
<p>Although the operation is fairly straightforward, there are many technical aspects to the procedure that may be done differently depending on the surgeons training, experience, and philosophy. The operation involves placing an implant that is made of silicone (either cohesive silicone gel or solid silicone) in the buttock area to improve size, shape and contour. In order to understand your options and the operation, let us go through the five decisions that will need to be made. The issues are the following:</p>
<ol>
<li><strong>Implant selection</strong> :The implant is either round or oval shaped (so-called anatomic implant). Like breast implants, it comes with either a smooth or a textured surface. The content of the implant can be either solid silicone or cohesive silicone gel. The most commonly used is a solid silicone implant. The fact that the implant is solid is excellent since it cannot rupture, spill, or deflate. The cohesive gel is not available in the United States . The decision as to which type of implant is used will be heavily influenced by your surgeon&#8217;s preference and philosophy. The augmented buttock has a natural feel, it can be somewhat firm to the touch, but it just feels like you have been working out for a long time.</li>
<li><strong>Incision Location</strong> : Once the implant has been selected the next task is to decide the type of incision to be used.The incision can be placed in several locations:</li>
</ol>
<ul>
<li>Superiorly on both sides of the upper buttock &#8211; Although the scars can be hidden in your undergarment or bikini, it is not recommended since the scar can be quite visible when not wearing clothing, and it requires two incisions.</li>
<li>Inferiorly in both of the gluteal creases where the &#8220;cheek&#8221; meets the back of the thigh. This choice will leave you with two scars that also can be visible.</li>
<li>One incision, which is, placed down the center of the sacrum &#8211; a small 4-6 cm vertical incision down the buttocks crease. This incision extremely well hidden and this approach only involves one scar. The only drawback is that it may have a higher infection rate than the other incisions. However, if this happens it is usually solved with antibiotics and wound care. If you choose this incision site you must be willing to follow your surgeons recommendations and at the first hint of infection contact your surgeon immediately.</li>
</ul>
<p><strong>Above or below the muscle</strong> : Once the type of incision has been chosen, the next step is to decide on the placement of the buttock implant. Will it be below the muscle? Or on top of the muscle?</p>
<ul>
<li>This again will heavily depend on your surgeon&#8217;s preference; many nice results have been attained with either position. However this will depend heavily on your anatomy.</li>
</ul>
<p><strong>To liposuction or not to liposuction or to Fat Graft :</strong></p>
<p>The final decision will be whether liposuction is to be combined with the procedure in order to add further contour to the buttock or possibly some fat grafting.</p>
<p>&nbsp;</p>
<ul>
<li>For some people just having an implant is not enough to create the desired results. So you surgeon may recommend either Liposuction or Fat Grafting along with the implants. Your surgeon will discuss this with you during your initial consultation.</li>
</ul>
<p><strong>Pre Surgery Instructions</strong></p>
<ul>
<li>Do not take aspirin or anti-inflammatory medications, Vitamin A or any blood thinning medications for 2 weeks prior to surgery. To see a complete list of medications click here</li>
<li>If you smoke, quit smoking for at least 1 month prior to your surgery (and anticipate no smoking for the recovery, smoking greatly increases your risk of complications)</li>
<li>Laboratory tests or blood work will be required several days prior. This checks your CBC for any present bodily infections, immune system deficiencies and general health.</li>
<li>You will also have a thorough examination by a cardiologist and xrays</li>
<li>Your personal assistant will help you fill prescriptions such as pain medication and antibiotics before surgery</li>
<li>You may want to consider purchasing any homeopathic medicines (i.e. Bromelain and Arnica Gel, for swelling and bruising) Be sure to discuss this with your doctor</li>
<li>Talk to your doctor about low-sodium foods to reduce swelling</li>
<li>Set up a hotel recovery area: lots of pillows, books, magazines, journal, stationery, lotion, baby wipes, tv, videos, remote control, etc.</li>
<li>Black out windows so you can rest during the day</li>
<li>Prepare icepacks to reduce post-op swelling</li>
<li>Laxatives (pain medications are often binding</li>
<li>DO NOT eat or drink anything after midnight on the evening prior to your surgery.</li>
<li>DO NOT bring rings, watches, or other jewelry or large sums of money to the the hospital; leave these items at safely at home.</li>
<li>If you bruise easily, notify the office immediately and certain medications will be prescribed for you prior to surgery to help prevent bruising.</li>
<li>Take Vitamin C capsules or tablets for approximately 3-4 weeks prior to surgery. This is necessary to promote good healing. You may continue this vitamin after surgery. [Also, Discuss with your doctor the use of SinEcch or Bromelain, a pineapple extract, for added anti bruising and anti swelling properties. As always discuss this with your surgeon before you take ANYTHING.]</li>
<li>Prior to surgery, should you develop any fever, chills or other signs of a cold or other infection, call the office immediately.</li>
<li>Morning of surgery, wash thoroughly with an antibacterial soap.</li>
<li>On the day of surgery, wear loose clothing which will be easy to get off and on after operation (consider a shirt that buttons in front)</li>
<li>Follow your physician&#8217;s directions carefully regarding medications, eating &amp; drinking, etc.</li>
</ul>
<p><strong>Possible Risks and Complications</strong></p>
<p>All surgery has potential complications. Although complications are not very common in cosmetic surgery, they do occur and when thinking about procedures such as buttock implants or any other surgical procedure you should be informed, before you decide on the surgery.</p>
<p><strong>Infections-</strong> Buttock Implants are less common than they probably should be considering the area of the surgery. Typically infections take place within 4 to 5 days and may require the implant to be removed for a few months. It is important to note infection could happen as late as one month after the surgery which would also require the implants to be taken out. Your surgeon will educate you about the symptoms of an infection. If you think you do have an infection. Call SSI or your surgeon immediately.</p>
<p><strong>Bleeding</strong>&#8211; This is possible as with any surgery. This in the worse case may require another operation to stop the bleeding although this is seldom required.</p>
<p><strong>Pain</strong>&#8211; Buttock implant is indeed a painful operation. The pain can last up to a month. Although ty.pically the first week is the worst.</p>
<p><strong>Nerve Injury</strong>&#8211; Small and large nerves could be injured during the operation but his is very rare. Although it is common for mild temporary numbness for the first few weeks.</p>
<p><strong>Swelling</strong>&#8211; Swelling is normal and may take longer than a month for noticeable swelling to subside.</p>
<p><strong>Skin Damage</strong>&#8211; It is possible that the overlying skin can become scarred or stretched. This may be temporary or permanent. This may also result if there is an infection.</p>

Your doctor will ask you to describe in detail what you would like improved. Be specific about what you would like done. Bring up all of your concerns no matter how small. This is your time to discuss your wants and desires so your surgeon fully understands your expectations. <p><strong>How much fat can you expect to retain after surgery?</strong></p>
<p>When fat is transplanted into areas that are not predetermined fat areas, like the lips, the chance of fat survival is nominal after the first implantation if no vascularity is introduced to the inner most areas of fat. This results in fat necrosis (fat cell death) which your body will safely absorb and your augmentation results, lessened. With this procedure, reportedly there is nominal fat necrosis/absorption.</p>

The surgery will last usually around 3 hours depending on how many areas are being liposuctioned to harvest the fat for the grafting. <p><strong>What to expect after the surgery and during your recovery?</strong></p>

A French surgeon, Dr. Illouv is the creator of the procedure known today as Liposuction. The earliest liposuctions were performed in a dry manner, with no added fluids. This was found to be unsatisfactory as there was heavy blood loss during surgery when removing the fat.<br />
Nowadays liposuction is performed using a wet technique (Tumescent or super humid), which means the introduction of a solution with adrenalin, prior to suction, sufficient to reduce the bleeding during the procedure. Liposuction is now the number one aesthetic surgical procedure performed in the world.</p>
<p><strong>Liposuction today : </strong>As the population has gotten larger and as plastic surgery is increasingly turned to as a means to correct weight issues, plastic surgeons have been taking ever increasing percentages of body weight from their clients. There are several schools of thought about how many liters of fat, liquid and blood can be taken without causing harm to a patient.</p>
<p><strong>How much fat is safe to remove?</strong></p>
<p>The American Society of plastic surgery and The Brazilian Society of plastic surgery opinions differ on this issue, but the average opinion is between 7 to 10 % of the total body weight. For the average patient that means around a gallon of fat, liquid and blood which equals generally less than 6 pounds total weight loss realized 2-3 months after surgery.</p>
<p>This is an average, but as you can see it is not a weight loss fix but a way to sculpt the body into a better shape.</p>
<p><strong>LVL, large volume liposuction</strong></p>
<p>For patients that want to have more than 7 to 10% of their body weight liposuctioned are categorized as Large Volume Liposuction (LVL). Large Volume Liposuction is a liposuction performed over the 7 to 10% limits, but with an increased risk to the patient. This is still Liposuction, but with a completely different set of safety requirements.</p>
<p>The trauma placed upon the body during LVL is very similar to the trauma a third degree burn victim may encounter. The patient loses large amounts of intravascular fluids which causes a sharp decrease in blood pressure and severe dehydration. In rare cases it has been reported that patients have gone into cardiac arrest during LVL due to the large loss of intravascular fluid.</p>
<p>To avoid these and other complications it is critical to have a senior anesthesiologist and surgeon that can manage the patient as an intensive care patient during and after the surgery with expansion fluids and blood transfusions.</p>
<p>Blood transfusion in LVL are performed as auto transfusion (using your own blood) to avoid blood related diseases. In a period of one or two months before the surgery blood from the patient is collected until there are 3 or 4 units for the surgery. With the auto transfusion we can take 15 to 20% of the body weight or 2 1/2 gallons of liposuction fluid. This must be performed in a hospital with a critical care unit for safety. LVL will involve two days in the hospital minimum for the patient.</p>
<p>Large Volume Liposuction is indicated to reduce major volumes of localized fat such as thighs and hips or other special cases. It is not safe to perform multiple areas LVL. LVL will not replace the personal commitment to loosing weight, good nutrition and exercise. Please note LVL, is not included in SSI&#8217;s Packages and is only available upon consultation with your SSI plastic surgeon.<br />

Inherited traits often prevent people from achieving their desired body shape. Even years of exercise may not yield significant results in certain people. For many, the answer is liposuction, also known as liposculpture, a procedure that extracts fat deposits from specific areas of the body.</p>
<p>A small incision is made in the skin just large enough to accommodate a small hollow tube called a cannula. The cannula suctions the fat from selected areas of the body to create the desired contour. This procedure is ideal for those who are within 30% of their ideal body weight and would like to contour specific areas.<br />
<strong>Liposuction</strong> is usually performed under general anesthesia and takes from one to 3 or more hours depending on the area and amount of fat being removed. Recovery also depends upon the extent of your surgery. Sutures are removed in one week. Normal athletic activities can be resumed in about three weeks.</p>
<p><strong>Tumescent Technique:</strong></p>
<p>The tumescent technique for liposuction is still the most commonly used method for achieving desired results in contouring areas such as the waist, hips, thighs and buttocks. This technique works by injecting controlled amounts of a special fluid under the skin. This fluid causes the fat to swell such that it is easier to remove.This fluid also numbs and constricts blood flow to the area to reduce blood loss. Because there is so little blood loss, there is minimal bruising.</p>
<p><strong>Medication</strong> is also added to this fluid such that post-operative pain is reduced for 18 to 24 hours after surgery. After the fluid is injected, a special hollow tube called a cannula is connected to a suction machine and then inserted under the skin. The cannula is moved back and forth to create thousands of tiny tunnels where the fat has been removed.</p>
<p>After surgery, a compression garment, or girdle, is put on the patient. The tiny tunnels collapse under the pressure of the garment. The garment is worn most of the time for the next 6 weeks.</p>
<p>The tissue continues to contract within the tunnel, creating a slimmer appearance. A new method, called Ultrasonic Assisted Liposuction, or UAL, is now being used in conjunction with tumescent techniques.</p>
<p><strong>Ultrasonic Technique:</strong><br />
A new method, called Ultrasonic Assisted Liposuction, or UAL , is now being used in conjunction with tumescent liposuction techniques. UAL works by converting ultrasonic energy into heat, literally melting the fat cells, allowing the remains to be suctioned away. This method reduces the amount of trauma to the tissue, since less force is needed by the surgeon, leaving fibrous tissue and vascular structures intact. There is minimal blood loss, less bruising, and better retraction of tissue.</p>

The concept of liposuction is surprisingly simple. Liposuction is a surgical technique that improves the body&#8217;s contour by removing excess fat deposits located between the skin and muscle. Liposuction involves the use of a small stainless steel tube, called a cannula (from the Latin word for reed, tube, cane). The liposuction cannula, typically connected to a powerful suction pump, is inserted into the fat through small incisions in the skin, and fat is removed by suction as the cannula creates tiny tunnels through the fat. During the healing process after liposuction, these tiny tunnels shrink and disappear, resulting in an improved body contour.</p>
<p><strong>2. Who is a good candidate for liposuction?(top)</strong></p>
<p>A good candidate for liposuction is defined as any patient who is likely to be happy with the results of liposuction. The best candidates for liposuction are in good health and have realistic expectations of what liposuction can accomplish. Liposuction can provide a good candidate with a significant improvement, but it is unlikely to achieve perfection. There is no definite age or weight limit for patients who are &#8220;good candidates&#8221; for liposuction. Many liposuction patients are of average size but are concerned about localized accumulations of fat. However, some of the happiest patients have been individuals who are somewhat obese. A good candidate usually has one or more localized accumulations of fat that can be removed by liposuction</p>
<p><strong>3. Who is not a good candidate for liposuction?(top)</strong></p>
<p>A person who expects absolute perfection is not a good candidate for liposuction. Excessively obese patients are usually not good candidates for liposuction. A person who has a serious medical problem is not a good candidate for liposuction.<br />
<strong>4. Is liposuction a reasonable treatment for obesity?(top)</strong></p>
<p>Liposuction is not a good treatment of obesity. Liposuction is not effective, even as a last resort, for people who are unable to lose weight by dieting and exercise. Obese patients almost always regain the weight that is removed by liposuction unless there is a dramatic reduction in calorie intake (by dieting) or a significant increase in calorie expenditure (by exercising). Whenever large-volume liposuction has been used in an attempt to treat obesity by surgery, there has been a significant increase in the incidence of serious surgical complications. It is not safe to remove huge amounts of fat by liposuction. It is dangerous to remove more than 8 to 10 pounds of fat by liposuction in a single day. Thus, liposuction will not be of any significant benefit for an obese patient who believes that liposuction will aid in the effort to lose weight.</p>
<p>On the other hand, an overweight person whose weight has been stable for many years and has certain problem-areas of fat may be a good candidate for liposuction. Liposuction in an obese patient is reasonable when the goal is to improve a troublesome body contour area. It is not reasonable to use liposuction as a surgical technique for weight loss.</p>
<p><strong>5. What is a successful liposuction surgery?(top)</strong></p>
<p><strong></strong><br />
A liposuction surgery is a success when the patient is happy with the results. The surgeon&#8217;s goal is to obtain an optimal aesthetic result rather than to maximize the amount of fat removed. One of the most common causes for disappointment in the results of liposuction surgery is the removal of too much fat, which produces an abnormal or unusual appearance. The cosmetic success of a liposuction surgery is often the result of removing an amount of fat equivalent to less than a pound or two of butter from a woman&#8217;s saddlebag area or from a man&#8217;s love handle area. While this amount of fat is relatively small compared to a person&#8217;s total body weight, it does produce a dramatic change in the patient&#8217;s silhouette.</p>
<p><strong>6. Does liposuction always remove cellulite?(top)</strong></p>
<p><strong></strong><br />
Liposuction improves the silhouette of the body, but does not necessarily eliminate the pre-existing subtle &#8220;puckering&#8221; of the skin that is often referred to as &#8220;cellulite.&#8221; Liposuction does reduce the degree of cellulite to a degree but it is unlikely to completely eliminate cellulite.</p>
<p><strong>7. Does liposuction produce permanent results?(top)</strong></p>
<p>After liposuction the body&#8217;s new shape is more or less permanent. If a patient does gain a moderate amount of weight after liposuction, then the figure will simply be a larger version of the new body shape. Fat cells that are removed by liposuction do not grow back. As long as the patient does not gain excessive amounts of weight, the new, more pleasing silhouette is permanent. Of course after liposuction the clock keeps ticking, and advancing age will produce the usual changes in the shape of the body associated with the aging process. If a person gains weight after liposuction, she/he will not accumulate as much fat in the treated areas as would have happened if liposuction had not been done.</p>
<p><strong>8. Does the fat come back in other spots after liposuction?(top)</strong></p>
<p>If a patient does not gain weight after liposuction, then fat does not accumulate in other areas of the body. However, if a patient gains a significant amount of weight, say more than 10 pounds (5 kg), after liposuction, then the fat must go somewhere. In fact, the fat accumulates in every area of the body in proportion to the amount of fat cells in each area. Areas where fat cells have been removed by liposuction will accumulate relatively little fat, while areas not treated by liposuction will collect relatively more fat. For example, if a woman gains weight after liposuction of her hips, outer thighs, and abdomen, then most of the fat will be deposited elsewhere such as the woman&#8217;s breasts, face, back and legs.</p>
<p><strong>9. Does liposuction cause dimpling or indentations in the skin?(top)</strong></p>
<p>Dimpling and indentations in the skin is a known risk of liposuction. However skin irregularities are unusual in the hands of a skilled surgeon. The use of large diameter cannulas tends to increase the risk of irregularities, while the use of microcannulas (less than 2.8 mm in outside diameter) reduces this risk. The use of microcannulas and multiple adits (very tiny 1.5 mm round holes placed in the skin) allows the surgeon to make a criss-cross pattern of tiny tunnels throughout the fat which produces smoother liposuction results compared to using larger cannulas and only two or three entrance incisions. When large cannulas are used, any inadvertent passage of the cannula too close to the skin may leave a depression or furrow, whereas one pass too close to the skin with a microcannula will not leave a visible depression.</p>
<p><strong>10. What Alternatives Are There to Liposuction?(top)</strong></p>
<p>Liposuction is never absolutely necessary. If you decide that liposuction is not for you, then you may consider the following alternatives to liposuction. Weight loss without liposuction can produce excellent aesthetic results. Weight loss can be achieved by dieting (decreased calorie intake) or by increased exercise (expenditure of calories). An alternative to abdominal liposuction is a tummy tuck.</p>

<p><strong>How is the Mini Tummy Tuck Performed?</strong></p>

Most tummy tuck procedures are performed under general anesthesia. In what many surgeons refer to as a full tummy tuck procedure, a surgical incision is made across the lower abdomen right above the pubic area. Following the creation of this initial incision, a second incision is made around the patient&#8217;s belly button as a means of loosening the surrounding skin. Next, the patient&#8217;s skin is separated from the wall of abdomen. Following this surgical maneuver, the surgeon pulls the loose abdominal muscles from the left and right flanks and sutures them together. This action results in tighter abdominal muscles, creating a stronger, firmer abdominal wall as well as a smaller waist. Directly after the tightening and suturing of the abdominal muscles, excess skin is removed and a new opening is made in the correct position for the belly button.</p>
<p>To complete the abdominoplasty, any remaining incisions are closed with sutures (stitches) and/or staples. Lastly, surgical gauze is placed over the incision area in an effort to keep the abdominal region clean and free from infection. A second option different from a full tummy tuck , and designed to treat problem areas existing below the belly button, is a partial tummy tuck . This less invasive form of abdominoplasty is frequently performed on an outpatient basis and typically requires a local form of anesthetic. Contingent on the patient&#8217;s desired outcome, as well as their pre-operative status, the surgeon may recommend that either a partial or full tummy tuck be done in conjunction with Liposuction . Liposuction is frequently incorporated into those tummy tuck cases where additional fat deposits need to be removed from the hips, resulting in more rejuvenated, dramatic body contour. Occasionally, liposuction alone may offer the best results. The most appropriate treatment options will be discussed during the surgeon-patient consultation. A tummy tuck generally requires about two to five hours, depending on the extent of the procedure (i.e., &#8220;full tummy tuck&#8221; vs. &#8220;partial tummy tuck&#8221;), the desired out come and the pre-operative condition of the patient. Typically, a &#8220;partial tummy tuck&#8221; takes about one to two hours. While many tummy tuck procedures are performed in a hospital setting, some are done in an office surgical suite. The location of where the tummy tuck procedure is to be performed will depend on the patient&#8217;s preferences as well as the surgeon&#8217;s. Furthermore, depending on the extent of the tummy tuck, an overnight stay in a hospital is common. This allows for the patient&#8217;s recovery process to be monitored by trained medical staff. If the treatment area is minimal, the patient may return home the same day. These issues are usually discussed and decided by the surgeon and patient during the initial consultation. While it may take most tummy tuck patients several weeks to feel completely back to normal, those patients who are in good physical condition will experience faster recovery times. Additionally, some tummy tuck patients are able to return to work after two weeks, others will require up to four weeks to recuperate. Many surgeons recommend that tummy tuck patients begin a program of light exercise following surgery. The motion afforded by this activity will reduce swelling, decrease the possibility of developing blood clots and enhance skin tone. Vigorous exercise, such as lifting heavy objects, etc., should be put off until it may be done comfortably and the surgeon has provided their consent to do so. As with most surgical procedures, scars do result from tummy tucks. Typically, surgical scars appear to worsen during the first few months following a tummy tuck &#8211; this is normal. In some tummy tuck cases it may take up to a year before most scars flatten out and lighten in color. While scars never disappear completely, tummy tuck scars are placed in areas that will subsequently be covered by clothes, bathing suits, etc.</p>
<p><strong>How Much Pain is Associated With a Tummy Tuck?</strong></p>
<p>For the first few days following a Tummy Tuck, expect some degree of swelling in the abdominal region. Furthermore, it is likely that the patient will experience some pain and discomfort which may be controlled by various pain medications. In an effort to reduce any pain and discomfort associated with a tummy tuck, it is common that two to three days of bed rest is recommended. During this time, the surgeon may suggest that the patient keep their legs slightly bent at the hips to reduce the amount of strain placed on the abdominal muscles. Furthermore, most surgeons will provide the patient with an &#8220;abdominal binder&#8221; which serves to offer support to the tightened areas. The surgeon will provide additional instructions for various activities such as showering, changing the surgical dressings and walking. It is frequently recommended that patients walk as soon as possible, even though they may not be able to stand straight up initially. Stitches and sutures are typically removed within the first five to seven days following surgery. Deeper sutures with ends that protrude through the skin, those used at deeper levels under the skin, will come out in two to three weeks.</p>

<p>Women frequently choose breast reduction because of severe back, neck and shoulder aches created by the large size of their breasts also the feeling that they are severely limited in what clothing they feel they can wear. The physical discomfort that is caused by very heavy, large breasts can limit participation in certain activities or exercise, In addition, breasts that are out of proportion to a women&#8217;s body can be a source of embarrassment and self-consciousness.</p>
<p>Breast reduction can be performed at any age on a healthy woman, but it is usually recommended to wait until the breasts are fully developed. <p>The surgeon removes excess glandular tissue, skin and fat, and moves the nipple and areola into their new position to match the breasts new contours. During the natural aging process, with weight gain and loss, and following pregnancy, the breasts support structures become stretched, leading to a change in the shape of the breasts and sagging.<br />
The breast lift, or mastopexy, encompasses several different surgical procedures designed to restore a woman’s breasts to a more youthful, firm, and shapely appearance. In a breast lift surgery the breasts are raised, the nipples are repositioned and the areolas (the darker skin around the nipples) may be reduced in size if you desire. If you have lost breast volume over the years and you want to add more fullness, implants may be inserted during breast lift surgery to increase breast size. Breasts can form a variety of different shapes which have been given different names. For example, some woman have an enlarged bulging nipple relative to the rest of the breast; this is commonly called a “snoopy” breast. A “tubular” breast refers to a breast with a narrow base and prominent breast tissue in the front of the breast, leading to a tube shaped breast. Asymmetry (one breast is different size or shape than the other) is another normal finding in women’s breasts that can be corrected with mastopexy.<br />
Mastopexy, with or without augmentation is designed to rebuild the support structures of the breast, reshape the breast, and remove excess skin to restore a more youthful appearance.</p>
<p><strong>Do I Need A Breast Lift?</strong></p>
<p>Well there are many different ways that you can determine whether you need a breast lift. Below we have put a few of your options but please realize that there is nothing that can substitute a consultation with a surgeon for a proper diagnosis.</p>
<p><strong>First method</strong></p>
<p>Essentially breast ptosis indicates that the breast is falling on the chest as well as rotating downward. Plastic surgeons usually divide ptosis into degrees to describe various amounts of the descent of the breast. The basic criteria for ptosis frequently accepted by most plastic surgeons looks at the relative position of the nipple areolar and the fold under the breast known as the inframammary fold. Remember that the fold is essentially stationary as the breast is falling. Therefore, if the nipple areolar is above the level of the IMF there is no sag.</p>
<p>When the areola is at the same level or slightly lower than the mammory fold this is known as first degree ptosis.<br />
If the areola is 2-3 cm below the fold this is second degree ptosis</p>
<p>If the areola is lower than 2-3 cm and pointing toward the floor (rotated 90o) this is third degree ptosis.<br />
Other important relative measurements include the distance from the notch in the top of the sternum (breast bone) to the nipple and the distance from the areola to the mammory fold.</p>
<p>There are two other conditions that need to be mentioned and distinguished from ptosis.</p>
<p>The first is an empty sac syndrome. Here there is a loss of volume of the breast while the skin envelope remains the same size. The breast appears empty and is often confused with ptosis, but the areola and mammory fold are in the correct relationship. The procedure to correct an empty sac syndrome is to fill the breast with an implant.</p>
<p>The second condition is called pseudoptosis. Here a large percentage of the breast volume is at or below the level of the areola creating a hollow in the upper part of the breast. In this condition the relationship of the areola to the mammory fold is acceptable and again the usual solution is an implant to acquire some volume in the part of the breast above the areola.</p>
<p>To calculate ptosis, a ruler is placed across the chest into the inframmary crease and the distance between the center of the nipple and the top of the ruler is measured. If the nipple is above or just over the ruler, this is called Grade 1 ptosis. Grade 2 ptosis is defined as the nipple measuring 1-3 cm below the ruler, If the center of the nipple is more than 3 cm below the top of the ruler, this is called Grade 3 ptosis. Pseudoptosis occurs when the nipple is above the mammary crease but the breast itself is flattened and droopy.</p>
<p><strong>When should I have a breast lift?</strong></p>
<p>A breast lift can be performed at any age but plastic surgeons usually recommend waiting until breast development has stopped. Pregnancy and breast-feeding may affect the size and shape of your breasts, so you may wish to postpone surgery until after having children. Still, many women elect to go forward with the surgery before having children, knowing that they can address any pregnancy-related changes later. The milk ducts and nipples typically are not affected during breast lift surgery, so usually the procedure will not alter your ability to breast-feed.</p>
<p><strong>Difference between Breast lift and Breast Reduction</strong></p>
<p>Breast lifts and breast reductions are two common plastic surgeries used for breast enhancement. Breast lift surgery and breast reduction surgery are actually very similar operations. Both the breast lift and breast reduction surgeries strive to raise the overall position of the breast mound and both also will reposition or lift the nipple and areolar tissue to a higher and more youthful position on the breast mound. The major difference between a breast lift and breast reduction is that a breast reduction surgery will reduce the overall breast volume and make the breast mounds smaller. A breast lift does not necessarily require that any breast tissue be removed. In fact, some breast lift operations are also performed while placing breast implants for breast enlargement. So a breast lift mainly is responsible for lifting the nipple and areolar position to a higher position, it does not imply a breast volume change. A breast reduction does lift the breast and does imply a breast volume change.</p>
<p><strong>How is a breast lift performed?</strong></p>
<p>Mastopexy can be performed with local or general anesthesia. The surgery can take anywhere from 1.5 to 4 hours under normal circumstances.<br />
Below are descriptions of different procedures that are used; note that there are many different names for similar procedures.</p>
<p>The Crescent procedure involves removing a crescent shaped piece of tissue from above the areola (from 10 o’clock to 2 o’clock) and then suturing the incision closed which lifts the nipple and breast upwards. This technique is often done during a breast augmentation and is only recommended for those women who have only a small amount of ptosis.<br />
The Benelli lift or Periareolar lift removes a donut-shaped piece of tissue from around the areola. Once this incision is made, the surgeon is able to tighten the breast tissue in all directions. The periareolar breast lift procedure can also enhance breast appearance by reducing the size of the areola and repositioning the nipple (slightly upward). A periareolar breast lift procedure is most appropriate for women with a small degree of breast and/or nipple malposition.</p>
<p>The Benelli-Lollipop uses the same technique as the Benelli lift but also includes an incision from the areola to the inframammary fold. This allows more skin and breast tissue to be removed and reshaped. This is for those who have medium ptosis, too much for the Benelli only and too little for a full anchor incision.<br />
The most common approach, the “anchor” incision, is actually three incisions. The first incision is made around the areola. The next runs from the bottom of the areola down to the breast crease and the third incision follows the horizontal breast crease. Working through these incisions, excess skin is removed from the lower portion of the breast. Areolas that have been stretched over time can be resized and the tissue is repositioned to reshape the breast. The breast tissue and nipple are lifted into the higher position, and the nipple is repositioned</p>
<p><strong>What is the recovery period like after a breast lift procedure?</strong></p>
<p>Immediately after surgery, your breast will be sore and swollen. You will have bandages, sometimes a drain (to drain excess fluid), and a supportive bra. You will need to wear a supportive bra twenty four hours a day for several weeks. Drainage of fluid and crusting is normal. You should be up the first day after surgery but most patients do not return to work for about a week. Strenuous activities, including sexual activities, should be avoided for several weeks. Your breast shape will appear flat at the bottom and full on the top for several weeks but will gradually take their natural shape.</p>
<p>After surgery your skin may be dry and the skin may appear to be thin. Your breasts may also have “tingling” sensations, occasional pain and loss of nipple sensation. Incisions need to be cleaned regularly and may become bright red and raised during the healing process but after 6 months to a year incisions should become less noticeable.</p>
<p>Depending on the surgeons technique of choice he may use internal sutures that will not need to be removed at any time and will slowy dissolve or he may use external sutures to close the incision which will need to be removed 2 to 3 weeks after the surgery.</p>
<p><strong>What are the risks of mastopexy or breast lift?</strong></p>
<p>There are multiple risks associated with mastopexy. It is critical to understand these risks and to have realistic expectations about the outcome of your surgery. Only through an in-depth discussion with your surgeon will you be able to understand the risks, benefits, and expected outcomes. Nipple necrosis leading to loss of the areola and/or nipple is a rare but serious side effect. Nipple necrosis is caused by impaired blood flow to the nipple and areola after surgery. The risk of this complication is higher in smokers. Asymmetry of the size, shape and level of the nipple is a common complication. It is impossible to create perfect symmetry and small differences should be expected. Major asymmetries can be corrected through an additional procedure. Inability to breast feed after breast lift can occur but many of the procedures used today usually do not affect breast feeding. The risk varies with the type surgery. Mastopexy can lead to changes in the skin sensitivity because removal of skin involves cutting through nerves that are in the skin. Often, sensation in the nipple returns over months to years. Fluid accumulation in the breast, called a seroma, can occur after mastopexy. Seromas can be drained and usually resolve within a few weeks. Accumulation of blood in the skin after surgery is called a hematoma and usually must be removed surgically. Hematomas can increase the risk of infection and may increase the scarring. It is important that you stop all over the counter, herbal, and prescription medications and vitamins that affect bleeding to decrease your risk of hematoma formation. Scarring occurs with any surgery and final scar appearance cannot be predicted. A scar can vary from a nearly invisible line to a large, raised, discolored scar. Unattractive scars can be treated through medical or surgical intervention. Infection is always a risk of surgery. You may be given antibiotics after the surgery and post operative wound care is important as well.</p>
